大腸癌取扱い規約第7版補訂版の要点

Introduction of General Rules for Clinical and Pathological Studies on Cancer of the Colon, Rectum and Anus（7th edition, revised version）

要旨　「大腸癌取扱い規約（第7版補訂版）」では，主として大腸生検組織診断分類（Group分類）に変更が加えられた．新 Group分類では，従来の異型度分類から診断分類へと概念が変化し，生検診断の際には診断名に併記される付加的なものとなった．Group 2は，第6，7版では“非腫瘍性”であったが，補訂版では“腫瘍性か非腫瘍性か判断の困難な病変”と定義された．Group 2の定義が変更されたことで，状況によってはGroup2と判定される病変に癌が含まれることが起こりうる．これは補訂版において最も留意すべき点の1つであり，病理医・臨床医ともに十分な理解と相互確認のうえで診療に望む必要があると思われる．

　In the General Rules for Clinical and Pathological Studies on Cancer of the Colon, Rectum and Anus（GR）（7th edition, revised version）, histological diagnosis classification of biopsy specimens（Group classification）was the main modification. In the new Group 2, the classification “non-neoplatic” in the 6th and 7th. The edition of the GR, is changed to “indefinite for neoplasia” in the 7th edition, revised version. By the fact that the definition of Group 2 is modified, cancer may be included in the Group 2 lesions when histological diagnosis is difficult. This is one of the important points which should be considered in the 7th edition, revised version. Therefore, it is necessary to obtain agreement on that modification between pathologists and clinicians including physicians, endoscopists, and surgeons for appropriate diagnosis and treatment.
