大腸ESDに対する工夫と進歩―高周波電源装置の条件設定と意義

Setting and Meaning of a High Frequency Unit for Endoscopic Submucosal Dissection

要旨　内視鏡的粘膜下層剥離術（ESD）が大腸腫瘍に対しても施行されるようになり，高周波電源装置はなくてはならない存在である．現在のところ安全性・有効性のエビデンスはなく，各エキスパートの経験に基づく設定が推奨されているに過ぎない．特に大腸腫瘍病変に対する切除は腸壁が薄く，管腔の条件によりスコープの操作性が制限されることなどもあり，高周波電源装置の条件設定を十分に理解する必要がある．当院ではバイポーラナイフ（Bナイフ）を使用している．バイポーラは切開部に電気が集中し生体抵抗も安定し低い出力で切開が可能であり，また深部に熱変性を来さないため安全に切除できる．

　Endoscopic submucosal dissection (ESD) for colorectal lesions is very difficult and the danger of complications such as bleeding and perforation is high. Because of this, the set up of the high frequency unit is important, and it is necessary to understand and utilize the principles in accord with which it operates. We use an endoscopic electrosurgical knife, the "B-Knife", that uses a bipolar current system to carry out safe dissection.
