 Endoscopic submucosal dissection (ESD) has made possible en-bloc resection of colorectal tumors regardless of size, except entirely circumferential lesions. The ESD technique for colorectal tumors is more difficult than that for gastro-esophageal tumors because of the anatomical and histological characteristics of the colorectal wall. For these reasons, its learning system is more advanced than the ESD technique for gastric tumors, and is accompanied with diagnostic training for colorectal tumors. Furthermore, this procedure has so high a rate of potential complications that surgeons have to be experts in endoscopic examination, risk management, and ethical judgement.

 The indications for ESD are as follows : First, tumors larger than 20mm in diameter. Secondly, pit patterns of type IIIL, IIIs, IV or VI tumors (mucosal or minimally submucosal invasive cancers). Thirdly, tumors without VN pit pattern or non-lifting signs. ESD was performed for 130 colorectal tumors (adenomas and early cancers) in 127 patients (76 male, 51 female, mean age of 65.9 years old) by using a Hook-knife. Among these lesions, 126 lesions (96.9%) were resected by the en bloc method. One case with complications due to perforation (0.77%) was seen, and was treated using endoclips. All cases were followed up after resection by colonoscopic examinations. Neither residual tumors nor recurrences were found. The safety of ESD for colorectal tumor has gradually increased. With the development of this device and the learning system, ESD will play an important role in therapeutic strategy for colorectal tumors.
