外科専属診療看護師（NP）による内視鏡外科手術助手の現況と信頼度調査

Current status and surgeons' trust in a surgical nurse practitioner（NP） as an assistant in endoscopic surgery

◆要旨：【目的】当院外科では2021年に診療看護師（NP）を採用した．本研究では，NPによる内視鏡外科手術助手の業務実態と，外科医による信頼度評価について検討した．【方法】2021年4月から2023年3月にNPが助手を務めた手術件数と時間を集計し，外科医によるNPと初期臨床研修医の手術助手技能および信頼度の比較評価を実施した．【結果】NPは2年間で194件中149件（76.8％）の内視鏡外科手術でスコピストを担当し，NPの評価は初期臨床研修医より有意に高かった（p＝0.03）．【結語】NPは安全かつ効果的に内視鏡外科手術助手を担い，外科医の負担軽減とタスク・シフト/シェアに貢献しうる．

【Objective】A surgical nurse practitioner（NP） was appointed in our department in 2021. This study aimed to investigate the NP's role as an assistant in endoscopic surgery and to evaluate the level of trust placed in the NP by surgeons. 【Methods】We retrospectively reviewed the number and duration of surgeries assisted by the NP between April 2021 and March 2023. Surgeons were surveyed to assess and compare the NP's technical skills and trustworthiness with those of junior residents. 【Results】During the study period, the NP assisted in 194 surgeries, including 149 endoscopic procedures（76.8％） as a scopist. The NP achieved significantly higher technical skill scores than the junior residents（p＝0.03）. 【Conclusion】Nurse practitioners can serve as safe and effective assistants in endoscopic surgery, supporting task shifting and reducing the workload of surgeons.
