腹腔鏡下に切除した骨盤内後腹膜に発生した単中心性Castleman病の1例

Laparoscopically resected unicentric Castleman's disease in the pelvic retroperitoneum : A case report

◆要旨：症例は40歳台，女性．他疾患の精査のため施行した腹部造影CTにて，偶発的に骨盤内後腹膜左側に35mm大の境界明瞭かつ辺縁平滑な腫瘤を指摘された．診断・治療目的に腹腔鏡下骨盤腫瘍摘出術を施行した．病理組織学的に単中心性Castleman病，硝子血管型と診断された．Castleman病は稀なリンパ増殖性疾患である．病変は後腹膜や，頸部，縦隔など多彩な部位に発生し，骨盤内に発生した場合には術前診断が困難な場合がある．骨盤内後腹膜に発生した単中心性Castleman病に対し，腹腔鏡下で安全に切除しえた．

　A woman in her forties underwent enhanced abdominal computed tomography（CT） for close examination of other diseases, which incidentally revealed a well-defined tumor 35×22mm in diameter on the left side of the pelvic retroperitoneum. The tumor was resected laparoscopically for a diagnosis and treatment. Histopathology revealed a diagnosis of unicentric Castleman's disease（CD） of the hyaline-vascular type. CD is a refractory, lymphoproliferative disorder and can occur at various sites, including the retroperitoneum, neck, and mediastinum. When tumors arise in the pelvis, a preoperative diagnosis is difficult. We herein report a case of unicentric CD arising in the pelvic retroperitoneal space that was safely resected laparoscopically.
