一過性に腸閉塞症状の改善を認めた鼠径ヘルニア偽還納に対して腹腔鏡下手術を施行した1例

A case of reduction en masse of inguinal hernia with transient improvement of intestinal obstruction treated with laparoscopic surgery

◆要旨：症例は55歳，女性．3か月前より左鼠径部の膨隆に気付き，自己整復を繰り返していたが，心窩部痛と鼠径部痛を伴い自己整復が困難なため受診した．受診後に自己整復を試み還納されたため経過観察入院となった．いったん腸閉塞症状の改善を認めたが，2日後に食事を開始した直後に腸閉塞の再燃を認め，CTにて鼠径ヘルニア偽還納状態であると診断し緊急手術を行った．一過性に腸閉塞が改善した鼠径ヘルニア偽還納は稀なため報告する．

　The patient was a 55-year-old woman. She noticed a bulge in her left groin three months earlier. She had repeatedly tried to return the hernia to its original position by herself, but as this was difficult due to the pain in her epigastrium and groin, she visited the hospital. After her visit, she managed to return the hernia to its original position by herself but was admitted to the hospital for observation. Although the symptoms of intestinal obstruction improved temporarily, they recurred two days later immediately after she started eating. A diagnosis of reduction en masse of inguinal hernia was made by CT, and emergency surgery was performed. We report on this rare case of reduction en masse of inguinal hernia, in which intestinal obstruction improved transiently.
