はじめに

　近年narrow band imaging（NBI）による内視鏡観察により咽喉頭領域の表在癌が指摘されるようになり，さらに経口的咽喉頭手術の発達により音声・嚥下機能の温存も可能になっている。中咽頭前壁病変は手術加療により鼻咽腔逆流をきたす可能性があるが，今回口蓋垂表在癌症例に対して内視鏡下咽喉頭手術（endoscopic laryngopharyngeal surgery：ELPS）の手技による切除を行い，鼻咽腔逆流を回避できた1例を提示する。

　Early laryngopharyngeal cancers can be detected with NBI endoscopy and treated transoral surgery. We report a case of uvula superficial carcinoma resected with endoscopic laryngopharyngeal surgery（ELPS） technique.

　The patient is 72-year-old male who had past history of esophageal carcinoma, oral floor carcinoma and hypopharyngeal carcinoma. Uvula superficial carcinoma was found under regular upper endoscopy, but he had no symptom. We resected the lesion by ELPS maneuver. Muscles of uvula could be preserved and the lesion was coated with polyglycolic acid sheets. He has no symptom of dysphagia and dysphonia after the surgery, and videofluorography showed no nasopharyngeal regurgitation.
