Abstract Purpose:This report presents a case of branch retinal vein occlusion(BRVO)in a transgender woman(MtF)taking ethinylestradiol(EE).

Case:A 66-year-old MtF had been receiving<100 μg of EE once daily for 30 years. She had visited a private eye clinic with blurred vision at the center of the right eye in January and was diagnosed with BRVO. She was referred to our hospital.

Findings:The visual acuity and intraocular pressure were 0.2(0.5)OD and 9 mmHg OD, respectively. No abnormalities were observed in the anterior or intermediate translucent areas. The blood pressure was 146/88 mmHg. Blood tests revealed no abnormalities. Fundus examination of the right eye revealed a retinal hemorrhage in the inferior part of the macular. Optical coherence tomography revealed cystoid macular edema in the inferior nasal area;however, the fovea was not affected.

Conclusion:We have experienced a case of BRVO in a patient over 40 years of age who developed BRVO while using oral EE. It is possible that the ease of thrombosis and hypertension caused by oral EE may have contributed to the onset of BRVO, but since this is a case report, we cannot make a definitive statement.
