Trichophyton tonsuransによる白癬の姉弟例

・Trichophyton tonsurans（T. tonsurans）による白癬は，柔道やレスリングなどの格闘技競技者に集団発生することが多い．・T. tonsuransは感染力が強いことと無症候性キャリアが多く存在していることが特徴であり，注意が必要である．・自験例は柔道を習っていた友人から弟に発症し，姉への家族内感染を生じたと考えられる．・小児の白癬症を診察した場合には，スポーツの経験の有無，家族内同症の有無などをとくに問診する必要があると考えられた．（「症例のポイント」より）

A familial case of dermatophytosis caused by Trichophyton tonsurans

Abstract A six-year-old boy visited our department for alopecia and red macules on the trunk. Physical examination showed alopecia with fine scales and black dots on the head. His sister developed erythema on the forehead. We carried out KOH examination, Sabouraud’s medium culture, and slide culture. Isolated fungi from the patient and his sister were identified to be Trhicophyton tonsurans with morphological features and direct sequencing analysis. We diagnosed them to be a familial cace of Trhicophyton tonsurans infection. Four-week therapy of oral itraconazole at a dose of 5 mg/kg was effective for the patient and topical antifungal therapy was effective for his sister．
