・姉と弟に同時にみられたMicrosporum canisによる頭部白癬を経験した.・母親に体部白癬,姉に顔面白癬の既往があった.・ペットの飼育はしていないが,友人宅の飼いネコに触れたという生活歴があった.・塩酸テルビナフィン(TBF)内服で症状の改善が得られないので,イトラコナゾール(ITZ)内服に変更し治癒した.小児の頭部白癬の場合,内服薬剤選択に苦渋することが多いが,治療に反応しにくい場合は薬剤の変更を検討すべきと考えた.また菌種の同定も治療のうえで重要である.(「症例のポイント」より)
Tinea capitis in the siblings caused by Microsporum canis

Abstract We report two cases of tinea capitis caused by M. canis. Eight-year-old sister had two hair loss patches with slight scales and itching. Six-year-old brother had a hair loss patch with infiltrative scaly erythema. The mother had a history of tinea corpolis a few months ago. The siblings often touched a cat at their friend’s house. They were treated with oral terbinafine(TBF), 62.5 mg/day, for 8 and 10 weeks respectively, but with no effect. So, with a change to oral itraconazole(ITZ), 50 mg/day, after six weeks the patch healed, and normal hair growth was detected. Tinea capitis always requires systemic treatment. It is important to determine oral medication with the knowledge of mycotic character.
