腎盂腎杯憩室は偶発的に認められる比較的まれな病態であり，腎嚢胞性病変として画像検査で発見されることがある。腎盂腎杯憩室から発生する尿路上皮癌は極めてまれである。今回我々は，嚢胞成分と石灰化成分を伴う腎悪性腫瘍の鑑別に，健診FDG-PET/CTが診断の一助となった腎盂腎杯憩室由来の尿路上皮癌を経験したので，文献的考察を加え報告する。

A calyceal diverticulum is a relatively rare condition and may appear as a cystic renal lesion on imaging. Urothelial carcinoma arising from a calyceal diverticulum is extremely rare. We herein report a case of urothelial carcinoma within a calyceal diverticulum, in which FDG-PET/CT was useful for differentiating a renal tumor with cystic and calcified components. When encountering a cystic renal mass with gross stones or milk of calcium, the possibility of urothelial carcinoma arising from a calyceal diverticulum should be considered.
