小脳血管芽腫はWHO分類Grade Ⅰの血管系の良性腫瘍である。その発生頻度は全脳腫瘍の中では約2％，後頭蓋窩腫瘍の中では約5～15％であり，成人の原発性小脳腫瘍の中では最多である。約30％はvon Hippel–Lindau diseaseと合併し，若年発生・家族性が多く，散発性は成人発生が多いといわれている^1）2）。今回我々は，術後13年目に髄液播種をきたしたまれな小脳血管芽腫の1例を経験した。臨床経過と小脳血管芽腫の髄液播種やMR所見について文献的考察を加えて報告する。

Cerebellar hemangioblastoma is a benign tumor of the vascular system classified as WHO Grade Ⅰ. It accounts for about 2％ of all brain tumors and approximately 5–15％ of posterior fossa tumors, making it the most common primary cerebellar tumor in adults. Here, we report a rare case of cerebellar hemangioblastoma with leptomeningeal dissemination occurring 13 years after surgery. We discuss the clinical course, leptomeningeal dissemination, and MRI findings of cerebellar hemangioblastoma, adding a literature review to our report
