気管支浸潤を伴った胸郭内の脱分化型脂肪肉腫の1例

A case of dedifferentiated liposarcoma of thoracic cavity with bronchial invasion

脱分化型脂肪肉腫が胸郭内に発生することはまれである。本症例では明確な脂肪成分がなく，石灰化を伴った巨大な腫瘤を形成し，気管支にも浸潤していたため，診断に難渋した。今回我々は，気管支浸潤を伴った胸郭内の脱分化型脂肪肉腫という大変貴重な症例を経験したため，文献的な考察を加えて報告する。

A 70-year-old man was referred to our hospital because he had difficulty in passing solid food for several months, followed by weight loss and pain ranging from the right hypochondrium to the right back. CT showed a huge mass mainly in the right thoracic cavity, infiltrating into the bronchus. A mass had internal necrosis and calcification, but no fatty component. CT-guided biopsy was performed, and pathology revealed a diagnosis of dedifferentiated liposarcoma. The diagnosis was challenging because dedifferentiated liposarcoma in the thorax is a rare disease, and there was no fatty component, and the tumor also invaded the bronchi. We report the case with some literature consideration.
