精索原発脱分化型脂肪肉腫の1例

A case of dedifferentiated liposarcoma of the spermatic cord

精索原発腫瘍は比較的まれな疾患であり，画像所見も非特異的な場合が多い。脂肪肉腫は四肢や後腹膜，体幹部に発生した場合は自覚症状に乏しく，巨大腫瘍として発見されることも多いが，精索原発の場合は表在性であるため比較的小サイズで発見されることがある。今回我々は，25mm大と小さなサイズで発見された精索原発の脱分化型脂肪肉腫を経験したので報告する。

The patient, a man in his 70s, presented with a 25 mm mass in the left spermatic cord on CT and MRI. He underwent high orchidectomy, and pathology diagnosed a primary dedifferentiated liposarcoma of the spermatic cord. Surgical resection is the first choice of treatment for liposarcoma, but local recurrence often occurs. Primary liposarcoma of the spermatic cord may be detected in a small size, and it is important to distinguish benign from malignant at the preoperative examination. If the mass extends beyond the spermatic cord fascia, as in this case, there may be reason to suspect malignancy.
