空洞形成を伴い緩徐に増大した細気管支腺腫/線毛性粘液結節性乳頭状腫瘍の1例

A case of bronchiolar adenoma/ciliated muconodular papillary tumor arising from left lower lobe of the lung

細気管支腺腫（bronchiolar adenoma：BA）/線毛性粘液結節性乳頭状腫瘍（ciliated muconodular papillary tumor：CMPT）は肺末梢に発生するまれな腫瘍性病変である。組織学的に粘液産生を伴い線毛細胞，杯細胞，基底細胞が乳頭状に増生することを特徴とする。予後は良好な疾患であるが，組織学的な良悪性の位置づけは明確に定まっていない。いくつかの画像所見が報告されているものの特異的な所見に乏しく，術前診断や原発性肺腺癌との鑑別は困難とされる。今回我々は，左肺下葉に発生したBA/CMPTの1例を経験したので，若干の文献的考察を加えて報告する。

Bronchiolar adenoma（BA）/ciliated muconodular papillary tumor（CMPT）is a rare tumor which occurs in peripheral lung, listed in the WHO Classification 5th edition in 2021. It is characterized by papillary proliferation of ciliated cells, goblet cells and basal cells with mucus production. BA/CMPT has a good prognosis, but its histological positioning of benign and malignant is not clear. Both imaging and histological findings make it difficult to differentiate it from primary lung adenocarcinoma. We report a case of BA/CMPT arising from left lower lobe of the lung, focusing on its imaging and histological findings.
