胸部外傷において胸部皮下気腫と鑑別を要する疾患として外傷性肋間肺ヘルニアがある。今回我々が経験した本症例は，CT画像において血管構造を伴う病変の胸壁外脱出を認めたため肋間肺ヘルニアと診断できた。またエコーにおいても経過観察を行い，保存加療にて改善した。比較的まれな疾患であり，文献的考察を交えて報告する。

Traumatic intercostal pulmonary herniation is one of the conditions that must be differentiated from thoracic subcutaneous emphysema in thoracic trauma patients. In the present case, the CT images showed a lesion with vascular structures protruding outside the chest wall, which allowed us to make a diagnosis of intercostal lung herniation.

The patient was followed up by echocardiography and improved with conservative treatment. This is a relatively rare disease, and we report the case with a discussion of the literature.
