異所性膵は日常診療でしばしば遭遇する疾患であるが，そのがん化（異所性膵癌）の報告は極めて少ない。今回，胃前庭部粘膜下の嚢胞性腫瘤として発見された異所性膵癌の1例について文献的考察を加えて報告する。

A 50-year-old man with persistent epigastralgia showed severe submucosal stenosis in the pyloric area by endoscopic examination. A 5-cm extramural cystic mass with subtle capsular thickening was visualized on CT at the greater curvature of the gastric antrum, whereas nearly complete obstruction was seen on fluoroscopic contrast radiographs. Either ectopic pancreatitis or cancer was considered as a preoperative diagnosis. Postoperative histopathological study verified adenocarcinoma arising from ectopic pancreas tissue in the gastric antrum.
