Ⅱ～Ⅲ期乳癌に対する術後強度変調回転放射線治療（VMAT）の初期経験

Initial experience of post-operative volumetric modulated arc therapy for Stage Ⅱ-Ⅲ breast cancer

乳癌において腋窩リンパ節転移が4個以上ある場合，または腋窩リンパ節転移が1～3個であっても腫瘍が5cmを超える場合や腫瘍占拠部位が内側である場合は，内胸リンパ節転移の発生率が20％以上と報告されている^1）。ハイリスク乳癌の内胸および鎖骨上リンパ節を含む術後照射は局所・所属リンパ節再発率を減少させることが示されている^2）3）。強度変調放射線治療（intensity modulated radiation therapy：IMRT）を用いて乳癌術後にリンパ節領域照射を行った報告では，呼吸器への有害事象を増加させることなく，安全に照射することが可能であった^4）。また，強度変調回転放射線治療（volumetric modulated arc therapy：VMAT）は，線量分布の向上と照射時間の短縮が期待できる^5）。VMATを乳癌術後の領域照射に用いた臨床報告は少ない。我々は術後照射における臨床的標的体積（clinical target volume：CTV）内の線量均一性の向上を意図してVMATを開始したので，初期経験を報告する。

Post operative radiotherapy using volumetric modulated arc therapy（VMAT）was done for 33 patients with stage Ⅱ-Ⅲ breast cancer. Internal mammary, supraclavicular, and axillary lymph node areas were included in the clinical target volume（CTV）, along with entire chest wall or conserved breast. Ninety-five to 105％ isodose volume could cover the entire CTV in all patients, and acute dermatitis was not significant. Also, no serious chronic cardiopulmonary adverse events were observed. It was postulated that VMAT is a safe and effective tool to deliver post-operative radiotherapy for patients with node-positive breast cancer.
