前立腺癌に対する外部放射線療法を通常照射法で行う場合，治療中または治療期間中の前立腺の動きや毎日の照射位置のセットアップ誤差を考慮して，10mm程度の計画標的体積（planning target volume：PTV）マージンを設定して，主に4門照射などによる3D-conformal照射が行われている（図1A）。この照射方法では比較的照射範囲が広く設定されるため，高線量領域から直腸を避けることは困難であることから直腸の有害事象を生じさせない程度の処方線量（70Gy程度）しか投与できず，十分な局所制御率が得られない。そのため，前立腺の位置の不確かさや，直腸，膀胱の前処置を考慮する必要はない。

A prostate move independently to the bony anatomy depending on the bladder and rectum filling. Therefore, precise preparation to the bladder and the rectum before each radiotherapy session impact on dose delivery accuracy. In order to achieve this, the occupations in the department of radiation oncology need to communicate with individual patients and understand patients’ life style.

Additionally, IGRT can detect some components of CTV variation, which was reported by ICRU report 62. However, IGRT system is not only used for image registration, but also should be used for calculation of the PTV margin, which determined using IGRT data.
