小児がん放射線治療における輪郭入力と線量とトレードオフ

Delineation, dose, and trade-off in pediatric cancer radiotherapy

小児がんは年間の発症数が2,000～2,500例の希少がんである。小児がんのうちおよそ25％は白血病が占めており，次いでリンパ腫，脳腫瘍が多く，それぞれ小児がん全体のおよそ2割を占める。その他の小児がんには軟部腫瘍，神経芽腫などがあるが，こういった腫瘍は頭頸部，胸部，腹部，四肢といった様々な部位に発症するものが含まれる。このように病理学的診断や発生部位が多様でありながら，まれな小児がんについては，個々の病状に応じた放射線治療を理解するとともに，小児がんの放射線治療の特殊性や放射線治療指針の開発の背景を知ることが有益である。こういった知識は放射線治療医が，成人がんの治療指針を小児がんの治療に外挿するときの留意点を理解したり，小児がんのみならず，がんの放射線治療の課題を解決したりするのに役立つ。

Radiotherapy for pediatric cancer can be characterized by extended target volume irradiation；it involves multidisciplinary treatment. Delineation of the target had been established in earlier periods of pediatric cancer treatment exploration and continuously implemented for over 20 years. For several years, optimization of delineation has become a key issue in pediatric oncology, along with advancing radiotherapy techniques and emerging adverse events as major issues of pediatric cancer treatment. Learning about developing delineation for various pediatric cancer radiotherapy will allow us to explore more effective multidisciplinary approaches for pediatric and adult cancers.
