心アミロイドーシスのMRI・CT診断

Diagnosis of cardiac amyloidosis by MRI and CT

心アミロイドーシスを適確に診断するためには，心エコーや心臓MRI，ピロリン酸シンチグラフィなどの画像検査による評価が欠かせない。心臓MRIは，心アミロイドーシスと臨床像が類似する他疾患（肥大型心筋症など）の鑑別に役立つほか，心アミロイドーシスの重症度評価や予後予測，病勢モニターに有用である。最近では，心臓CTを心アミロイドーシスの評価に活用する方法も報告されている。心アミロイドーシスの診断には，各種画像検査の特性を十分理解したうえで，本疾患の診断ストラテジーにこれら画像検査を適切に組み入れて活用していくことが重要である。本稿では，心アミロイドーシスにおけるMRIとCTによる診断のポイントについて，最近の知見をふまえて概説する。

Cardiac MRI is useful in differentiating cardiac amyloidosis from other cardiac diseases such as hypertrophic cardiomyopathy, which produces cardiac hypertrophy. Cardiac MRI is also useful in assessing the severity of cardiac amyloidosis, predicting prognosis, and monitoring disease progression. Recently, the use of cardiac CT as an alternative to cardiac MRI in the evaluation of cardiac amyloidosis has been reported. For the proper diagnosis of cardiac amyloidosis, it is important to incorporate and utilize these imaging modalities appropriately in the diagnostic strategy of the disease, based on a thorough understanding of the characteristics of each imaging modality.
