腎粘液管状紡錘細胞癌（mucinous tubular and spindle cell carcinoma：MTSCC）はWHO 2004分類から独立した疾患として記載され，報告も増えている。2004年のWHO分類では低悪性度（low grade）と記載されたが，2016年のWHO分類で低悪性度という記載は削除された。MTSCCは低悪性度が典型であるが，今回，高悪性度（high grade）と診断されたMTSCCを経験したので報告する。

Mucinous tubular and spindle cell carcinoma（MTSCC）is a rare subtype of renal cell carcinoma that is typically a low-grade tumor. We report a case of high-grade MTSCC with metastasis. The tumor showed a prolonged enhancement on CT/MRI that was typical of MTSCC. However, preoperative diagnosis was difficult because the tumor had little high T2 signal intensity area that was suggestive of “mucin” as a typical MTSCC. Both radiologically and pathologically, the cranial and caudal sides of the tumor showed different features.
