ニボルマブ関連二次性硬化性胆管炎が疑われた1例

A case of nivolumab-related secondary sclerosing cholangitis

ニボルマブは免疫チェックポイント阻害薬の一つで，ヒト型抗ヒトprogrammed cell death-1モノクローナル抗体である。2014年に悪性黒色腫に対し薬価収載され，現在では非小細胞肺癌，腎細胞癌，再発または難治性の古典的ホジキンリンパ腫，再発または遠隔転移を有する頭頸部癌など次々に適応が広がっている。しかし，免疫チェックポイント阻害薬は，T細胞活性化作用による過度の免疫反応に起因する免疫関連性有害事象（immunerelated adverse events：irAE）を惹起する。irAEは皮膚炎，肝機能障害，腸炎，甲状腺疾患などが比較的頻度が高く特に留意する必要があるが，その他の臓器にも障害が及ぶことがある。我が国のニボルマブ市販後調査では胆管炎や胆嚢炎も少数ながら報告されており，2017年7月にはニボルマブの使用上の注意の改訂として，重大な副作用に硬化性胆管炎が追加された。今回，我々はニボルマブ導入後に二次性硬化性胆管炎を疑われた1例を経験したので，若干の文献的考察を加えて報告する。

We reported a case of 73-years-old-woman with nivolumab-related secondary sclerosing cholangitis. She was malignant melanoma, who was treated with nivolumab, and presented with high levels of biliary tract enzymes of a blood sample, one year and 9 months after first administration. It showed diffuse wall thickening of the bile duct and gallbladder, extrahepatic bile duct dilation without obstruction. Nivolumab-induced cholangitis was suspected and she received steroid therapy. However, both levels progressed, in addition characteristic bile duct changed with multifocal strictures and segmental dilatations.
