門脈輪状膵は膵実質が門脈や上腸間膜静脈を全周性に巻き込み癒合した形態異常である。通常は無症状であるが膵頭十二指腸切除術において膵頭部の切離に注意を要することや術後膵液瘻のリスクが高いことから，解剖学的特徴を理解しておくべき形態異常である。今回，我々はまれなretroportal main pancreatic ductを呈する門脈輪状膵を経験したので，若干の文献的考察を加えて報告する。

Circumportal pancreas is a rare congenital anomaly resulting from the fusion of the pancreatic parenchyma around the portal vein/superior mesenteric vein. Patients with this anomaly are generally asymptomatic. However, it is important to discover this anomaly before pancreatic resection to avoid the risk of inadvertent pancreatic injury and postsurgical pancreatic fistula. Besides, our case was accompanied by retroportal main pancreatic duct in which the main pancreatic duct runs behind the portal vein.
