がん診療におけるIVRの未来

The future of interventional radiology in cancer care

いきなり私事で恐縮だが，「この先どうなるかなんて誰にもわからない」が私の口癖である。数年程度の先なら多少は予想がつくかもしれないが，20年先のことなど誰にもわからない。だから，楽しいのかもしれない。かつて結核が人類にとって最大の病魔であった時代，優秀な若い医師はこぞって結核撲滅のための道に進んだらしい。その当時も，あまり効果のない薬剤をどう組み合わせ，どう投与したらよいかなど，今流にいうなら多剤併用についての臨床試験が一生懸命行われたという。ところが，ストレプトマイシンなどの優れた抗結核剤が出現し，状況が一変した。結果，彼らは，職を失わないまでも，別の領域に進まざるを得なくなったという。そんな時代に「結核診療における○○○の未来」なんて原稿を書く人がいたかどうかは知らないが，ある意味，この原稿もそんな無謀な企てかもしれないと感じている。ただし，がんはどうも一筋縄ではいかないようで，ゲノム医療が劇的に進歩しつつあるとはいえ，いきなり状況が大きく変わることはなさそうにみえる。しかし，兎にも角にもまずいのは，「予測は勝手でも，それが当たっているか外れているかは未来には必ずバレる」という点で，この原稿も20年後に「バカなことを書いて」と笑われる可能性大である。そう考えると，地震予知をする方々が発言に慎重になられるのもよくわかる。ま，地震予知に比べれば社会への影響はとても小さいし，この文章でIVR医の人生が変わる可能性も多分ないだろうから，責任は軽いかもしれない。それでも，後世の笑い者になるのも愉快ではないので，多少は歴史的流れを俯瞰して，「がん診療における未来」について書いてみようと思う。降水確率10％でも「念のため傘を持っていけば困らない」くらいの軽い気持ちでお読みいただきたい。

From the viewpoint of history of Interventional radiology（IR）in cancer care, the following points seem important to brighten the future of IR. 1）Brush up sedation, analgesia and anesthesia to make IR a truly minimally invasive treatment, 2）IR physicians keep the perspective from the practices for patients care, 3）Clinical trials for making evidences and listing up IR on guidelines, 4）Development and improvement of imaging equipment and devices, 5）Getting the adequate approvals of the indication of IR devices and the covering by insurance system, 6）Education and environment setting for well and fitting management of imaging devices and devices, 7）Knowing the movement of other specialties and the collaboration with them, 8）Knowing the change of social situation in cancer medicine and raising the awareness of IR. Don’t wait for the coming of future, but we should make the future of IR. With overcoming these issues, the brighten future of IR in cancer care will come.
