本稿では2019年2月9日に開催された放射線治療談話会勉強会（放射線性皮膚反応のケア）における講演内容に基づき，放射線皮膚障害の病態の基礎を解説する。筆者は緊急被ばく領域での医療・研究に従事している皮膚科医であり，事故による被曝，すなわち “放射線1回照射後の皮膚反応” について述べている。放射線治療で行われている分割照射による臨床症状とは異なるものの，放射線皮膚障害への理解の一助となることを期待する。

Cutaneous radiation injuries progress over time, and their severity increases with radiation dose. Early and secondary erythema depends on the functional changes in the blood vessels. Massive death of the stem cells in the basal layer causes the main clinical manifestations of dry and moist desquamation. The ulcers may be developed not only by necrosis, but also by injury of blood vessels. The progressive vascular damage causes the third wave of erythema, leading to the appearance of new ulcerations. Chronic injuries are characterized by the skin atrophy, fibrosis, and teleangiectasia.
