トリアージ訓練における傷病者役看護学生の体験からの学び

Experiential Learning of Students Simulated Casualties in Triage Practice

　本研究の目的は、トリアージ訓練において傷病者役を体験した看護学生の学びを明らかにすることである。トリアージ訓練終了後に傷病者役学生にアンケート調査を行い、自由記載を分析した結果、【傷病者への気づかいの重要性】【全身を観察することの重要性】【トリアージ実施者としての態度】【傷病者の気持ちの理解】の4カテゴリーが明らかになった。学生は傷病者役の視点だけでなく、トリアージ実施者の視点からどのような行動が求められているかについて学びを得た。災害看護教育への示唆として次のことが見出された。学生がトリアージ実施者と傷病者役の両方の立場を体験あるいは実施者と傷病者役の体験を共有でき、段階的に学べる教育方法を検討する。トリアージの場面においても、主体的に行動でき、傷病者を尊重した態度や自己の役割を考えて行動できる力を養う教育が重要である。今後、人間の尊厳を基盤としたヒューマン･ケアリングを実践するトリアージの方法論を構築していく必要がある。

Abstract

The purpose of this study was to clarify students' experiential learning through triage simulation. Following qualitative evaluation of a student questionnaire(completed after triage practice), four themes emerged:

1)students became more aware of casualties' appreciation for individual care and attention; 2) students became more aware of the need to observe the patient holistically, through all five senses; 3)students gained the need attitude for triage nurses, and 4)students became aware of the need to understand the suffering of the casualties.

Through these experiences, other issues were raised, among them the need for triage nurses to take initiative and act independently. This activity also provided an opportunity for students to experience the situation from the viewpoint of the patient, enabling them to further develop awareness of triage nursing requirements.

This study suggests that triage education should involve experiential learning from the viewpoints of both the carer and the cared for. In addition, future task design should incorporate methodologies that promote awareness-raising of the needs listed above.
