嚥下チーム立ち上げにおける諸問題―経験の浅い言語聴覚士が中心となる場合

Starting a dysphagia rehabilitation team The role of the Speech-Language-Hearing Therapist (SLHT) in starting a dysphagia rehabilitation team : SLHTs with little experience

　リハビリテーションにおけるチームアプローチの重要性は周知のところであるが，摂食・嚥下障害に関するリハビリテーションでは，高度なリスク管理が必要とされ病棟や栄養部など他職種との連携が特に重要となる．またチームの構成員は，知識・認識を共有することが必要である．近年摂食・嚥下障害に関する取り組みを始める病院・施設において，経験の浅い言語聴覚士が嚥下リハビリの始動すべてを任せられることが多い．しかし，経験の浅い言語聴覚士が嚥下チームを立ち上げることは容易なことではない．本稿では，筆者自身の一般病院での経験について述べる．そこから得たチーム作りの糸口について整理し，最後に嚥下チームの立ち上げにおける言語聴覚士の役割の1つといえるコーディネーターの仕事について考えてみることにする．

　In rehabilitation, a team approach is important.　In dysphagia rehabilitation, highly developed risk management is necessary, and cooperation with other professionals such as hospital nursing staff and dieticians is crucial.　Team members need to share a common knowledge base and common judgment criteria.　In recent years, SLHTs with relatively few years of experience have often been put in charge of starting dysphagia rehabilitation teams in hospitals and at other facilities.　However, this is not an easy task for inexperienced SLHTs.　I describe my experience in a general hospital and, based on this experience, discuss issues related to starting a team and to the role of the SLHT as a team coordinator.
