肝疾患に対する胸腔鏡下アプローチについて，諸家の報告と併せ，肝腫瘍に対する胸腔鏡下RFAを例に挙げ，われわれの行っている手術手技を中心に述べた．胸腔鏡下治療の適応は，経皮的あるいは腹腔鏡的治療が困難な右横隔膜下病変，特に腫瘍であり，主としてマイクロ波凝固療法，およびラジオ波焼灼療法が施行されている．本術式は，確実かつ低侵襲な治療を目的として，症例ごとに最適なアプローチ法を選択する上で，重要なオプションとなると考えられた．今後，根治性を高めるためには，病変の正確なナビゲーションが最も重要な課題であり，機器の開発・改良とともに，さらなる手術手技上の工夫が期待される．
We reported our technique of thoracosopic treatment for liver diseases, mainly focusing on the thoracoscopic radiofrequency ablation to liver tumors. Thoracoscopic therapy was indicated for the tumors located beneath the diaphragm (segment 8 or 7 of the liver), where it was hard to approach with the percutaneous or laparos-copic techniques. These tumors are currently treated with the microwave coagulation or radiofrequency ablat-ion.Because of the less invasiveness, thoracoscopic techniques may be one of the important surgical approaches to the management of hepatic tumors.
