区域切除術は元来，炎症性肺疾患の手術として用いられてきたが，近年は肺癌の治療においても縮小手術の1つとして受け入れられてきており，積極的な治療としての可能性も検討されてきている．本手術を胸腔鏡下に行うことはまだ一般的ではないが，技術的な可能性は高いと思われ，本稿ではわれわれが行っている手技を供覧する．胸腔鏡下では立体的な解剖の認識にやや困難性があり，また手術操作上にも困難性がある．したがって，本手術を行うためには解剖の十分な理解と胸腔鏡下での手術操作の習熟が必要である．本手術は現時点ではcompromised hostに対する縮小手術と認識されているが，今後は標準手術の1つとしての発展が期待される．
Segmental resection of the lung was first developed for inflammatory lung diseases. Recently, it has been ac-cepted for limited resection of lung cancer. More recently, it has been applied for radical surgical procedures. Although video-assisted segmental resection is not prevalent, its technical feasibility is promising. Our practice is therefore presented. Due to several problems, such as somewhat difficult stereo-tactic recognition or incon-venient operative techniques, sufficient understanding of thoracic anatomy and mastery of thoracoscopic surgi-cal skills are necessary.
