肺癌に対する根治的VATS肺部分切除術の適応と工夫

Wedge Resection of Lung Cancer Using Video-assisted Thoracoscopic Surgery (VATS):Problems of Indications and Techniques

　本稿では，筆者らの肺癌に対する根治的VATS肺部分切除術の適応，手技，成績を示し，その問題点と対策を論じた．適応には積極的適応および妥協的適応があるが，特に前者では腫瘍のサイズ，占拠部位とともに，高分解能CTにおけるground-glass opacity（GGO）比率を考慮して本術式を選択すべきである．切除組織の病理学的検討から，10mm以下のHigashiyama分類IV型腺癌が最も安全に本手技にて完全切除され，術後成績も良好であった．これらの症例はGGO比率が100％を呈することから，本分類は術式を決める術前因子として重要である．また，VATS部分切除時には腫瘍部位の同定のためのCTマーキングや腫瘍局所遺残のチェックとして肺切離面迅速洗浄細胞診を行うなど，根治性を十分に確保するための工夫が必要である．

We described our indications and techniques of wedge resection of lung cancer using video-assisted thoraco-scopic surgery (VATS). Not only tumor size and location but also ground-glass opacity (GGO) area on preop-erative high-resolution CT (HRCT) was informative for deciding intentional indication of this surgical modality.
