症例はステロイド療法に不応性の特発性血小板減少性紫斑病の47歳，男性で，脾臓摘出術目的で入院した．血小板数4,000／μl，出血時間30分以上で，術前に免疫グロブリン大量投与を行ったが血小板数の上昇を認めず，血小板10単位投与後の血小板数の半減期は約4時間であった．術中，術後の出血リスクを回避するために，ハンドアシスト法を用いた腹腔鏡補助下脾臓摘出術を行った．脾門部血管処理は腹腔外で行った．本術式により，開腹下手術より小さい開腹創のもと，従来の腹腔鏡下脾臓摘出術よりも確実な膵脾脱転と脾門部血管処理を行うことができ，術後経過は良好であった．本術式は従来の腹腔鏡下脾臓摘出術に比べて，操作性・安全性に優れた有用な術式であると考えられた．
The patient was a 47-year-old male with idiopathic thrombocytopenic purpura intractable to the steroid therapy. He was admitted to our department to perform splenectomy. The level of platelet was 4,000/μl, and the bleeding time was more than 30 minutes. Pre-operative mass administration of immunoglobulin was not effective to increase the level of platelet, and the half-time of platelet was ca. 4 hours even after transfusion of 10 units of platelet. To avoid the risk of intra-and post-operative bleeding, we performed laparoscopy -assisted splenectomy using hand-assist method.
