今日の内視鏡外科の普及は消化器外科における腹腔鏡下胆嚢摘出術に始まった．その低侵襲性のために患者サイドの要求から急速な広がりを見せ，外科医の意識にも変革をもたらした．機器の進歩とともに多くの消化器外科手術に応用され，いまや内視鏡外科を導入していない分野は皆無といえる．良性疾患は手術ロボットなどの機器の進歩と手技の工夫により大半は本手技の適応となると考えられる．早期癌ではその対象疾患が広がりつつあるが，進行癌に関しては機器のさらなる開発と気腹が癌に与える影響などの基礎研究が急務である．21世紀には消化器外科領域でどのような低侵襲治療が出現するか大いに楽しみである．
Laparoscopic surgery is now accepted as a minimal invasive procedure, providing less pain, aesthetic improvement and quicker return to normal activities. With the developement of techniques and instruments, various types of abdominal surgery have been performed under laparoscopy to improve QOL after surgery. Nation-wide survey revealed 97,000 operations including 95,000 cholecystectomies, carried out during the 7 years, from 1990 to 1997. With the future developement of the novel devices, most of the patients with benign diseases would be treated in a safe and effective way.
