要旨：縦隔の囊胞性疾患は比較的稀な疾患である．造影CTスキャンやMRIにより，縦隔囊胞性疾患は術前にほぼ正確な診断ができるようになってきた．縦隔囊胞は比較的切除の容易なものから，周囲臓器との癒着などにより切除の難しいものまで手術の難易度はさまざまである．本稿では比較的頻度の高い気管支囊胞，心膜囊胞，胸腺囊胞について手術適応および胸腔鏡下手術の実際について述べた．これらの疾患に対して胸腔鏡下手術はよい適応であると考えられるが，手術は安全で低侵襲であることが要求される．
Cysts of the mediastinum are relatively rare lesions encountered in the mediastinum. Computed tomography scan and magnetic resonance imaging reveal the cystic nature of the lesion. Although mediastinal cysts are relatively easy to resect, in some cases, the resection of the cyst is very difficult due to strong adhesion to adjacent organs of the cyst.
In this article, we described the operative indications of mediastinal cysts and the surgical technique and procedure of video-assisted surgery（VATS）for these lesions, especially of bronchogenic cyst, pericardial cysts, and thymic cysts. Although VATS is the good choice of treatment for mediastinal cysts, VATS should offer safety and less invasiveness for benign mediastinal cysts.
