われわれの行っている腹腔鏡下直腸前方切除術における安全で確実な切離再建の工夫を紹介する．一般的な内側アプローチによる腸管の剝離・授動，郭清，血管処理の後，恥骨結合直上に約5.0cmの縦切開を加え開腹し，ラッププロテクターレギュラーなどを装着する．8.0号ゴム手袋をラッププロテクターに装着し気腹する．ゴム手袋の指先端第1・5指を切開し一方より曲がりドアイヤン鉗子を挿入，腫瘍肛門側で直腸切離ライン口側を直腸に直角，腹背方向にクランプする．直腸内腔をイソジン生食にて洗浄後，エンドリニアステイプラー60mmを今一方の切開した指先端より挿入し，腸鉗子に平行に直腸に装着しファイヤーする．肛門側より自動吻合器を挿入しIO吻合を行う．
Washout of the distal rectum is widely accepted for preventing cancer implantation at the anastomotic site of the low anterior resection for rectal cancer. In case of laparoscopic rectal surgery, this conventional procedure has been more complicated because of lack of ideal bowel clamping methods. We use curved Doyen forceps through midline incision covered with Lap Protector and rubber glove under pneumoperitoneum for rectal wall cross-clamping. This procedure is as secure as open surgery for washout of the distal rectum preventing cancer implantation. Furthermore, it enables IO double stapling technique with endoscopic linear stapler in the same way as open surgery.
