閉塞性大腸癌に対し経肛門的イレウス管での減圧後に施行した腹腔鏡下大腸切除術の3例

Three cases of laparoscopic surgery for obstructive colorectal cancers after decompression with a transanal ileus tubes

◆要旨：近年，閉塞性大腸癌に対する，経肛門的イレウス管での減圧後の一期的切除・吻合術の有用性が報告されている．本法は，人工肛門回避と術後合併症の軽減が期待され，当院でも積極的に施行してきた．最近，筆者らは本法を腹腔鏡下手術に応用し良好な術後経過が得られた3症例を経験した．本法により，閉塞性大腸癌症例にも腹腔鏡下手術を安全に施行しうるだけでなく，さらなるquality of lifeの改善が期待できる．

　Recently, usefullness of single-stage resection after decompression by using a transanal ileus tube for obstructive colorectal cancer was reported. This strategy improves quality of life by avoiding colostomy and reducing complication. We experienced three cases of conducting single-stage resection and anastomosis laparoscopically after transanal decompression, which provided even better QOL. As long as the transanal decompression is successful, laparoscopic surgery can be performed safely for obstructive colorectal cancer.
