肥満症における腹腔鏡下胆嚢摘出術

　腹腔鏡下胆嚢摘出術（LC）は，著しい肥満は適応外とされている．肥満症における適応について，1992年7月より3年6か月間に当科でLCを施行した144例を対象に，標準体重より20％以上の23例を肥満群として検討した．手術操作で開腹移行となった症例は肥満群では1例で，非肥満群では3例であったが，肥満群に視野不良，脂肪過剰による剥離困難例はなかった．手術時間は肥満群152.5±44.2分，非肥満群140.0±37.5分と差はなかった．術後合併症は肥満群では認めず，非肥満群は出血の2例に再手術を行い，1例に無気肺を認めた．入院期間は肥満群10.7±5.2日，非肥満群10.9±3.3日と差はなかった．LCは肥満症といえども安全に施行することができ，積極的に適応すべき手術手技と思われた．

Laparoscopic cholecystectomy (LC) is now widely considered the treatment of choice for symptomatic cholelithiasis. Although morbid obesity has been con-siderd a relative contraindication to LC, few analyses have been reported on this issue in Japan. 144 LCs performed at our institution was analyzed if this rela-tive contraindication led to increased morbidity, in-creased rate of conversion to the open cholecystectomy,longer operating time, or longer hospital stay. Conver-sion to open cholecystectomy was nessessary in one patient with obesity because of technical difficulty.
