 A 59-year-old man presented with a left inguinal protrusion and pain. A CT scan showed left internal inguinal hernia. Transabdominal preperitoneal repair was performed, establishing a diagnosis of left external supravesical hernia. A 74-year-old man presented with a right inguinal protrusion. A CT scan showed bilateral internal inguinal hernia (R>L). Transabdominal preperitoneal repair was performed, establishing a diagnosis of bilateral external supravesical hernia with bilateral external and left internal inguinal hernia. We have great difficulties in diagnosing external supravesical hernia in which the hernia orifice is located on the inside of medial umbilical fold before surgery because it is rare. However, laparoscopic surgery makes it possible to diagnose and treat external supravesical hernia accurately compared with conventional anterior approach. Laparoscopic surgery is useful for the proper diagnosis and treatment of external supravesical hernia.
