Communicating “bad news”in the ICU

集中治療の現場では重症患者の診療を行っているため，医療者側から患者や家族に「悪い知らせ」を伝えることや，goals of care discussion（GOCD）は日常的に行われている。米国では，集中治療医療従事者のためのコミュニケーションスキルトレーニングが，よりよいGOCDを可能にするというエビデンスが数多く存在している。本稿では，そのトレーニング法の1つとして知られるVitalTalkについて紹介し，集中治療に携わる医療従事者が「悪い知らせ」を上手に伝え，患者の価値観を尊重したGOCDを行うための手法を解説する。

In the intensive care unit, where critically ill patients are treated, goals-of-care discussions （GOCDs） are routinely held, with clinicians communicating “bad news” to patients and their families. In the United States, there is a large body of evidence that communication skills training for intensive care clinicians can enable better GOCD. This article introduces VitalTalk, one of the communication skills training methods, and explains how intensive care clinicians can successfully communicate “bad news” and conduct GOCD that respects the patient's values.
