PSAと聞いて，前立腺の採血値の話ではなく，麻酔に関連する話題を想起する読者はどの程度いるであろうか。わざわざINTENSIVIST誌「麻酔」特集を手に取っている読者には馴染みのある話題かもしれないが，麻酔をバックグラウンドにもつ集中治療医よりも救急をバックグラウンドにもつ集中治療医のほうが，馴染みがあるかもしれない。本稿で話題としているPSAとは，処置時の鎮静・鎮痛procedural sedation and analgesia（PSA）である。本稿では，PSAの概念を概説し，シナリオに沿ってその注意事項・薬物に関して解説する。

When you hear the word “PSA,” does it remind you of a topic related to anesthesia rather than a prostate specific antigen? It may be familiar to readers of INTENSIVIST “Anesthesia”. However, the concept of “PSA” may be more familiar to intensivists with a background in emergency medicine than to intensivists with a background in anesthesiology. The PSA discussed in this paper is “Procedural Sedation and Analgesia” （PSA）. In this paper, the concept of PSA is outlined and the associated precautions and medications are explained along with the scenario.
