9．ICUにおけるspecial populationsに対する薬物投与量の考え方—肝機能障害，AKI，肥満患者

医薬品の添付文書に記載されている薬物投与量は，標準的な体格を目安に設定されたものである。慢性腎臓病（CKD）の患者ではその腎機能に応じて推奨される投与量が示された資料があるが，一方で，急性腎障害（AKI）の患者に対しての薬物投与量について示されたものは少ない。また，臓器障害がなくとも，肥満やるいそうという体格の問題に直面しても，その投与量の調節法について明確に記載された資料はないのが現状である。高リスク薬を使用することが多い集中治療領域において，このようなspecial populationsに対して有効かつ安全に薬物投与設計を行うために必要な知識について述べる。

Drug doses listed in the appendix are based on standard body size. Some recommendations are based on altered renal function in patients with chronic kidney disease, while there are few recommendations for patients with acute kidney injury. Even in the absence of organ damage, there are currently no clear descriptions of how to adjust dosages when confronted with the distinction of obesity versus lean body mass. In intensive care medicine, where high-risk drugs are often used, we will discuss the knowledge necessary for effective and safe dosage decisions for special populations.
