下気道の完全閉塞（気管腫瘍，気道異物，大量気道出血，等）および気管/主気管支の離断症例には，urgent V-V ECMOの適応がある。ECMO導入には主に盲目的挿入，X線透視またはX線写真ガイド下の挿入，およびエコーガイド下挿入の3つがあるが，経食道エコーを含めたエコーガイド下のECMO cannulationが，迅速性と安全性とを両立させやすい。urgent V-V ECMOは，大腿静脈経由の下大静脈脱血および内頸静脈経由の右房送血を基本とする。最近は1本のカニューレを挿入するのみでV-V ECMOを確立できる，bicaval dual-lumen cannula（AVALON）が使用可能であり，urgent V-V ECMOでの使用報告が散見されている。

There is an indication for urgent use of V-V ECMO in case of complete lower airway obstruction or transection. There are three ways for ECMO cannulation, including 1. blind, 2. fluoroscopy-guided, and 3. echo-guided. Echo-guided cannulation using transesophageal echo-cardiograhpy is superior in terms of safety and speed. The basic cannula position in urgent V-V ECMO is performed with the drainage cannula inserted in the femoral vein and positioned in the IVC, and the return cannula inserted in the internal jugular vein and positioned in the right atrium. The Bicaval Dual-Lumen Cannula（AVALON）has recently been made available, and there are reports of using double lumen cannulae in urgent V-V ECMO.
