気道確保は救急医療における最も重要な手技の一つである。気道確保を適切に行うためには，気道の解剖・生理に関する知識が不可欠である。そして，体位による換気や喉頭展開への影響を知る必要がある。

　本稿では，基本的な気道の解剖・生理の知識を整理するとともに，さまざまな体位での気道の開通性や喉頭展開の視野について文献を交えて解説し，現時点で最も気道確保に有利な体位を考察する。さらに困難気道とされる肥満患者における最適の体位についても言及する。

Maintenance of the airway is one of the most important procedures in emergency medicine. To maintain the airway, you need to know the anatomy and physiology of the airway, and the effects of the head and neck position on ventilation and laryngoscopy. In this column, we will review the anatomy and physiology of the airway, consider the effect of various head and neck positions on airway patency and the laryngoscopic view, and then discuss the ideal position to maintain the airway. We will also discuss the ideal position for obese patients who often have a difficult airway.
