急性腎傷害（AKI）や持続的腎代替療法（CRRT）を施行する状況下での栄養療法は，原疾患に影響されるところが大きく，これらの病態が代謝に与える影響を加味して決定される。基本的にAKIは安静時消費エネルギーに影響を与えず，これらの病態でも，必要エネルギー量は25〜35kcal/kg/日でよく，その投与を確実に達成するためにも，間接熱量測定による調節が望ましい。また，タンパク質投与も制限せず，喪失量を補充する意味で1.5g/kg/日以上を摂取することが求められる。特にCRRT中はタンパク質の喪失が増加するため，さらなる投与が必要とされる。AKI患者やCRRT施設下では，推算式による栄養目標の設定や栄養効果の判定はいずれも不確実であり，病態を理解したうえでの注意深い主観的包括的評価subjective global assessment（SGA）が求められる。

The optimal nutrition support for patients with Acute Kidney Injury （AKI） is an open issue. AKI often complicates underlying conditions and multiple organ failure. The need for nutrition support should be determined mainly by the nature of the underlying disease. Daily recommended energy requirements are between 25 and 35kcal/kg，and indirect calorimetry is recommended to match actual needs. At least 1.5g/kg of protein is needed daily with additional protein to compensate for losses. In particular，patients undergoing continuous renal replacement therapy have more protein losses. Conventional formulas to predict nitrogen balance and rapid protein turnover are not precise enough to estimate actual needs in these patients. To assess these patients，a subjective global assessment with careful inspection is essential.
