4．感染性心内膜炎—「診断のポイントと治療・合併症のマネジメント」の実際

感染性心内膜炎は臨床的に非常に多様であり，特異的な所見に乏しく，しばしば見逃されてしまう。そして見逃されてしまうと，やっかいな合併症に悩まされ，最悪生命にかかわる場合もある。そのため，診断をするためのポイントや検査を知っておく必要がある。また治療においては，心臓手術が必要な症例や重症合併症により全身管理が必要となる場合もあり，ICUでの管理を要する状況もしばしば経験する。微生物ごとの抗菌薬治療に加えて，外科手術の適応やタイミングを含めた治療戦略も把握しておく必要がある。

　2015年に米国心臓協会（AHA）がガイドライン^1）を改訂した。本稿ではこのガイドラインに基づき，臨床で議論になるであろうClinical Questions（CQ）を挙げながら，感染性心内膜炎の診断と治療について述べる。

The management of infective endocarditis （IE） is challenging. Unfortunately, clinical symptoms and findings of IE are often non-specific, making it difficult to diagnose some cases. A delay in the diagnosis and treatment of IE may result in complications. Thus, clinicians should be aware of important clinical features and diagnostic tests. When administering treatment for IE, some patients may require heart surgery, and severe complications could result, requiring management in the intensive care unit. Therefore, in addition to antibacterial treatment strategies for each microorganism, physicians should be aware of treatment strategies for surgical indications and timing, and the importance of working together with cardiologists and cardiac surgeons, infectious disease specialists, neurologists, and neurosurgeons. In 2015, the American Heart Association （AHA） revised its guidelines for IE. Here, we describe how to diagnose and treat IE based on the AHA guidelines while referring to clinical questions.
