16.理学療法士から見たPICS—PICSの評価とリハビリテーションの可能性

Physical therapists' perspective of postintensive care syndrome

ICU患者においては，ICU滞在中から運動機能・認知機能・精神の予後が悪化する集中治療後症候群（PICS）が生じることが問題となってきており，その予防が課題となっている。PICSは，ICU滞在中あるいは退室後に生じる運動機能，認知機能，精神の障害と，患者家族への精神的影響を含む概念であり，PICSの予防策の1つとして早期リハビリテーションが注目を浴びている。しかしながら，どのような患者に，いつから，どのようなリハビリテーションを行うことが，PICSの予防や治療に有用かはいまだ不明な点が多い。本稿では，理学療法士の立場からPICSの評価とリハビリテーションの可能性について考えてみたい。

With recent medical advances, the number of survivors of critical illness is increasing, and postintensive care syndrome （PICS） is a growing concern in public health. PICS is defined as a new or worsening impairment in cognition, mental health and physical function after critical illness. Early rehabilitation may be effective to prevent PICS. While there is accumulating evidence regarding the safety and efficacy of early rehabilitation in the intensive care unit, demonstrating the effectiveness of early rehabilitation to prevent PICS awaits further investigation. This article reviews and discusses early rehabilitation in the intensive care unit with respect to the prevention of PICS.
