脳卒中は，虚血性の脳梗塞と，出血性の脳内出血とくも膜下出血に大別される。脳卒中の原因としては，高血圧や脂質異常症，糖尿病などの動脈硬化性疾患，また，脳梗塞であれば心房細動などの明らかな塞栓源を有していることが多い。しかし，臨床現場ではそういった危険因子がない脳卒中の症例に出会うことがある。また，頭蓋内外の心血管疾患や全身性疾患に関連した脳卒中もある。本稿では，虚血性脳卒中と出血性脳卒中について基本的な診断の流れを提示しながら，どういった鑑別を挙げながら特殊な原因による脳卒中を診断していくのかについて解説する。また，それぞれの特殊な疾患について基本的な疾患概念や治療について述べる。

●脳梗塞はTOAST分類をもとに原因を確定して診断する。

●標準検査で潜因性脳梗塞と考えられたものは，さらに精密検査を行い原因を検索する。

●脳内出血において，皮質下出血の場合，出血が多発する場合，高血圧の既往がない場合については，MRIや脳血管造影などで原因精査を行う。

●脳動静脈奇形ではSpetzler-Martin分類をもとに治療方針を検討する。

●もやもや病では二次性の原因がないかを検索する。

Strokes are classified into two main categories: ischemic and hemorrhagic. The majority of strokes are due to atherosclerosis of intracranial or extracranial arteries, occlusion of small vessels and emboli from cardiac or arterial origins in patients with identifiable underlying conditions, such as hypertension, dyslipidemia, diabetes mellitus, and atrial fibrillation. However, approximately 25% of patients who have strokes have no such underlying conditions. In these patients with occult diseases, however, uncommon causes including paradoxical emboli through a patent foramen ovale, coagulation disorders, infection, paraneoplastic and inflammatory disorders, and vascular malformations such as Moyamoya disease or other arteriovenous malformations may exist. This review outlines the standard workup for patients with ischemic and hemorrhagic strokes, including magnetic resonance imaging or computed tomographic imaging of topographic features of the infarct or hemorrhage in the brain, magnetic resonance or computed tomography angiographic assessments of arteries in the neck and the brain, echocardiography, and 24-hour Holter monitoring. Subsequently, additional investigations for uncommon causes of stroke are discussed, including evaluation for atherosclerotic and nonatherosclerotic arteriopathies with catheter angiography, evaluation for cardiac sources of embolism with transcranial doppler monitoring for emboli, implant cardiac telemetry, and coagulation tests. An overview of the epidemiology, etiology, clinical presentations, diagnostic testing and treatment for these uncommon causes of stroke is given.
