NON-INSULIN-DEPENDENT diabetes mellitus (NIDDM), which affects 10 to 12 million Americans over the age of 20 years, is a complex disorder characterized by increased resistance to insulin and impaired secretion of insulin and associated with an increased risk of coronary heart disease, peripheral vascular disease, kidney failure, and blindness. The strongest predisposing factors for NIDDM are obesity and a family history of diabetes.

Along with proper diet and weight reduction, exercise has been advocated for the management of NIDDM. Whether physical activityis effective in preventing diabetes is unknown, but several indirectlines of evidence support the concept that increased physical activityhas a protective effect. First, physically active societies have lessNIDDM than less active societies, and as populations have becomemore sedentary the incidence of NIDDM has increased. Second,physical activity increases sensitivity to insulin, and regular endurance exercise induces weight loss and improves glucose tolerance.Third, greater physical activity has been associated inversely with theprevalence of NIDDM in several cross-sectional studies. Understanding the relation of physical activity to the occurrence of NIDDM iscomplicated further by the fact that high levels of physical activityare often associated with a low prevalence of other risk factors forthe disease, particularly obesity.
