Detection and clinical significance of multidrug-resistant organisms：macrolide-resistant Mycoplasma pneumoniae

　肺炎マイコプラズマ(Mycoplasma pneumoniae)には，その菌体内ではプラスミドのような外来の遺伝子が機能しない，リボソームのオペロンが1組しか存在しない，などの特徴がある．また，本菌は直接的な細胞傷害性は弱く，その疾患発症においては，細胞膜に含まれるリポ蛋白による炎症性サイトカインの誘導，あるいは細胞質に含まれる様々な物質による免疫の修飾など，宿主側の要因が強く関与している．したがって，適切な治療が行われなければ重症化するほかの病原体における薬剤耐性化とは様相が異なり，菌自体の薬剤抵抗性が必ずしも重症化には直結していない．本稿ではマイコプラズマの生物学的特性を踏まえたうえで，耐性菌問題について解説する．

Mycoplasma pneumoniae has several peculiar biological characteristics distinct from other bacteriae；extrinsic genes such as plasmids do not function within M. pneumoniae cells, and M. pneumoniae has only one operon for ribosome synthesis. While M. pneumoniae has only limited cytotoxicity involving excessive production of superoxides within infected cells, immunological abnormalities elicited by its membrane components or cellular components play a major role in its pathogenicity. Therefore, in M. pneumoniae infection, drug-resistance of the bacterium in itself does not necessarily lead to serious clinical threat, which is a quite unique aspect different from drug-resistance in other bacteriae.
