小脳橋角部動静脈奇形の1例—その神経耳科的検査所見の意義

I．はじめに

　小脳橋角部に発生する腫瘍およびその類似疾患の中で，聴神経腫瘍，髄膜腫，類上皮腫などはその代表的なものであり，動静脈奇形（Arteriovenous Malformation：以下AVM）などの血管奇形が発生することはきわめて稀である。しかしAVMは脳内に発生する血管奇形としては動脈瘤についで多く，かつ頭蓋内出血を惹起する重要な原因の一つ^1）でもあり，できるなら出血前の発見が望ましい。

　最近三叉神経痛を主訴として来科した患者の小脳橋角部にAVMを発見し，神経耳科的検査を併せ施行した結果，病巣部位を想定しうる所見が得られた。

　A case of arteriovenous malformation located at the left cerebellopontine angle region in a 64-year-old male was reported. Although he had never complained of vertigo except for some tingling feeling at his left cheek, neurotological findings indicated a possible central lesion in the posterior fossa. CT scan and angiography disclosed a dural arteriovenous malformation at the left cerebellopontine angle.

　Significance of the neurotological findings obtained in this case was also discussed.
